Family Sign-Up Form

Our requirements include: (1) that you live in a rural area within 20 miles of Lodi, Ohio, with no other Christmas program nearby; (2) that you have not applied to any other program for help with Christmas; and (3) that the children in your home are 15 years of age or younger.

* If you have children under 15 and over 15, you may include your older children, however they must be living in the home and still be attending high school. It will be up to your sponsor to decide if they want to purchase gifts for a child over 15.

Applications must be received by Sunday, November 15th, 2026. Families will only be considered on a case-by-case basis after this date.

Please explain, with as much detail as you can, why you are seeking assistance this year and how this program will benefit your family. There is a limited number of families we are able to help, and therefore need to focus on those who need it most.

Program Acceptance: Completing this form does not guarantee acceptance into the Hope for the Holidays program. Hope for the Holidays will try to assist as many families as finances allow, and I understand my family may not be chosen.Other Assistance & Eligibility: Until I am told that my family is not accepted into this program, I will not apply to another gift assistance program. I understand that if I fail to notify other agencies and accept assistance elsewhere, my family will permanently forfeit our eligibility for all BSGO programs. I understand that partners may also choose to terminate my family's eligibility for services they offer.Verification with Other Agencies: I authorize BSGO representatives to check with other gift assistance programs in the area to ensure my family is not also registered with them. This may include, but is not limited to: Bucky Cares, Community Action (CAWM), Spencer Village/Pantry, Matthew 25, Salvation Army, People to People, Good Samaritans, Love Inc., Toys for Tots, The School Districts, and Local Churches.Wish List & Sponsorship: I give permission for my family's wish-list to be shared with potential sponsors. This list will include ages, sizes, needs, and likes but no names or identifying information.Gift Responsibility & Liability: I understand that it is my responsibility to ensure gifts are age-appropriate, in working order, and used properly. I release from liability all BSGO program providers, partners, and volunteers and am grateful for the assistance.
